New Tax Form 1095-C for 2015

Man signing financial paperworkWe would like to inform you of a new form you will be receiving for the 2015 tax year, along with your usual W-2.

The form number is 1095-C, and it is issued in order to prove medical insurance coverage. This form must be used when preparing your taxes for 2015.

The form will note your name and information, along with each person you have covered under your medical insurance through The Macomb Group. It will include each and every month that each person was covered under Blue Cross Blue Shield, as well as your share of the insurance premiums.

The amounts on the form are strictly for informational purposes and will not be taxed.

If you worked for another employer for part of 2015, you will receive a 1095-C from them as well, noting the months you were covered under their plan.

* Please note that if an individual in your household was without medical insurance coverage at any time in 2015, you must pay a fee called the individual shared responsibility payment. You can get more details about the fee at the HealthCare.gov website.

The 1095-C form will help you avoid that fee, so watch for it along with your other documentation in January.
